A prospective cohort study.=20 J Clin Epidemiol 2002 Nov;55(11):1088-94.=20 The object of this article was to determine the predictive value of r= isk factors for recurrent falls and the construction of a fall risk model a= s a contribution to a mobility assessment for the identification of communi= ty-dwelling elderly at risk for recurrent falling in general practice. The = design was a prospective cohort study (n =3D 311). There were four primary = health care centers. A sample stratified on previous falls, age, and gender= of community-dwelling elderly persons aged 70 years or over (n =3D 311) wa= s taken from the respondents to a mail questionnaire (n =3D 1660). They wer= e visited at home to assess physical and mental health, balance and gait, m= obility and strength. A 36-week follow-up with telephone calls every 6 week= s was conducted. Falls and fall injuries were measured. During follow-up 19= 7 falls were reported by 33% of the participants: one fall by 17% and two o= r more falls by 16%. Injury due to a fall was reported by 45% of the faller= s: 2% hip fractures, 4% other fractures, and 39% minor injuries. A fall ris= k model for the prediction of recurrent falls with an area under the curve = (AUC) of 0.79, based on logistic regression analysis, showed that the main = determinants for recurrent falls were: an abnormal postural sway (OR 3.9; 9= 5% Cl 1.3-12.1), two or more falls in the previous year (OR 3.1; 95% Cl 1.5= -6.7), low scores for hand grip strength (OR 3.1; 95% Cl 1.5-6.6), and a de= pressive state of mind (OR 2.2; 95% CI 1.1-4.5). To facilitate the use of t= he model for clinical practice, the model was converted to a "desk model" w= ith three risk categories: low risk (0-1 predictor), moderate risk (two pre= dictors), and high risk (>/=3D3 predictors). However, a global consensus does no= t exist for elderly subjects who live independently in the community. The a= im of the study was to evaluate the benefit of an exercises program.MATERIA= LS AND METHODS: A control group (153 subjects) and an "Intervention" group = (150 subjects) were tested before and after a 1-year prospective study. The= "Intervention" group performed ten training sessions of physical activity,= based on balance, muscular activity and coordination. We compared the inci= dence of falls, and the performances in several tests between the two group= s.RESULTS: The clinical features were similar between the two groups: mean = age 71 years, 83% of females, subjects who were independent for activities = of daily living. However, all subjects presented risk for factors for falls= : 38% were not able to maintain the unipodal position more than five second= s, 29% had already fallen during the previous year. For the "Intervention" = group, the comparison of the performances before and after the physical act= ivity program showed significant increases (p < 0.001) for all the tests, a= nd specifically for the unipodal position and for the exercises performed w= ith eyes closed. After one year follow-up, the incidence rate of falls was = lower in the "Intervention" group compared with the control group, but the = difference was not significant (p =3D 0.32). However, falls occurred signif= icantly latter in the "Intervention" group (p =3D 0.02).CONCLUSION: Ten tra= ining sessions of physical activity allowed to improve the performance of e= lderly people in several tests. A group concerns 16 s= mall disability off drug parkinsonian patients, a group concerns 16 healthy= witnesses. All the persons benefited from a posture recording by means of = a force platform and were followed during 1 year.RESULTS: Data analysis und= erlines three groups of persons corresponding to three postural patterns, i= ndependently of the presence of Parkinson disease. A group (n =3D 18) did n= ot contain fallers, the second (n =3D 10 ) contained 20% of fallers, the th= ird (n =3D 4) contained 100% of fallers. Differences between the groups wer= e identified on 16 posturographic parameters.DISCUSSION: A group has a good= functional value and one does not record any fall. Its characteristics, wh= ich correspond to a category of persons who compensate well for the phenome= na of ageing, are found in the literature. A group has an intermediate func= tional value and regrets 20% of fallers. Kinetic profile reveals a tendency= to the stiffness of the posture. This group is going to operate rather ank= le strategies. A group has an inferior functional value and regrets 100% of= fallers. Kinetic profile seems disrupted and not to be able to adapt itsel= f in a satisfactory way to the situation otherwise than by stereotypical re= actions. A análise é do especialista em contas públicas Raul Velloso e ajuda a entender por que, embora os servidores públicos contribuam com percentual significativo do seu salário para a aposentadoria, o rombo das contas não pára de crescer. A tese de que os servidores não contribuem para ter benefício integral é questionável. Quem se aposenta pelo INSS não recebe mais do que R$ 1,561,56, mas durante a vida profissional seu desconto para a previdência fica limitado a 11% desse valor, mesmo que ganhe dez vezes mais. Com os servidores é diferente. Na União, por exemplo, o desconto de 11% incide sobre o salário integral, ou seja, quem ganha R$ 6 mil paga R$ 660 para a previdência. Levando em conta que o governo - patrão - também deveria contribuir no mínimo com o mesmo valor (empresas privadas pagam 20% da folha), a alíquota total ficaria entre 22% a 30%. Um cálculo atuarial elaborado para o JB pelo superintendente de Desenvolvimento de Produtos de Previdência da Sul América, Nelson Brasys, mostra que, para garantir uma aposentadoria integral pelas regras adotadas para quem ingressou no setor público a partir de 1998 (30 anos de contribuição e 55 anos de idade), no caso de uma mulher, seria preciso uma contribuição de 21,8% do valor do benefício. Para os homens (35 anos de contribuição e 60 de idade), o percentual cairia para 12%. Os cálculos levam em conta juros de 8,5% ao ano. Comparadas com o desconto atual, as projeções indicam que o problema não está na alíquota. - O grande problema do setor público não são os novos servidores, e sim a massa de pessoas que, depois da Constituição, conseguiu aposentadoria integral sem ter contribuído ou tendo pago só sobre o teto do INSS. Essa distorção é que precisa ser corrigida com a cobrança de uma contribuição adicional dos aposentados e quem ainda vai se aposentar - argumenta Raul Velloso. O projeto apresentado pelo PT não prevê mudança para quem já é aposentado e propõe regras de transição para quem já é servidor, com contagem proporcional do tempo trabalhado antes e depois da mudança. While not all sleep changes are pathological in later life, severe= disturbances may lead to depression, cognitive impairments, deterioration = of quality of life, significant stresses for carers and increased healthcar= e costs. The most common treatment for sleep disorders (particularly insomn= ia) is pharmacological. The efficacy of non-drug interventions has been sug= gested to be slower than pharmacological methods, but with no risk of drug-= related tolerance or dependency. Physical exercise, taken regularly, may pr= omote relaxation and raise core body temperature in ways that are beneficia= l to initiating and maintaining sleep. OBJECTIVES: To assess the efficacy o= f physical exercise amongst older adults (aged 60 and above). Bibliographies= of existing reviews in the area, as well as of all trial reports obtained,= were searched. Experts in the field were consulted. SELECTION CRITERIA: Ra= ndomised controlled trials of physical exercise for primary insomnia where = 80% or more of participants were over the age of 60. Participants must have= been screened to exclude those with dementia and/or depression. DATA COLLE= CTION AND ANALYSIS: Abstracts of studies identified in searches of electron= ic databases were read and assessed to determine whether they might meet th= e inclusion criteria. Data were analysed separately depending on whether re= sults had been obtained subjectively or objectively. MAIN RESULTS: One tria= l, including 43 participants with insomnia, examined the effectiveness of e= xercise in a population within an elderly population. At post-treatment, sl= eep onset latency improved slightly for both men and women. Total sleep dur= ation, sleep onset latency and scores on a scale of global sleep quality sh= owed significant improvement. Improvements in sleep efficiency were not sig= nificant. In some cases improvements indicated falls to below what are usua= lly considered pathological levels but the wide confidence intervals and sm= all sample size indicate that these findings must be interpreted with cauti= on. REVIEWER'S CONCLUSIONS: When the possible side-effects of standard trea= tment (hypnotics) are considered, there is an argument to be made for clini= cal use of alternative treatments in the elderly. We examined the effects of recombinant human growth hormone (GH) trea= tment on body composition and physical performance in elderly patients unde= rgoing elective total hip replacement. METHODS: We recruited 33 patients (a= ged 60-82 years) scheduled for hip replacement. The study was double-blind = and placebo-controlled with parallel groups. GH or placebo was administered= for 14 weeks preoperatively (target GH dose 0.04 U/kg/day) and 4 weeks pos= toperatively (with dose doubled for first 2 weeks postoperatively). Serum I= GF-I, body composition, mid-thigh muscle cross-sectional area, muscle stren= gth and 4-min walking distance were measured at baseline, 2-3 days preopera= tively and 4 weeks postoperatively. RESULTS: Mean serum IGF-I doubled durin= g GH treatment. Lean body mass increased preoperatively by an average of 5.= 2% (representing 1.8 kg) with GH treatment (P < 0.05 vs. placebo), but fell= postoperatively by 3% in both groups. Similar changes pre- and postoperati= vely were seen for mid-thigh muscle cross-sectional area, such that there w= as no net loss in the GH group but a fall of more than 10% in the placebo g= roup (P < 0.005 vs. GH). There was a trend towards a favourable effect of G= H on strength in the majority of muscles tested, but this was significant o= nly for the abductors of the nonoperated hip (P < 0.02), where there was a = 7% increase in strength over the whole study compared with a 25% decrease i= n the placebo group. Four-minute walking distance improved postoperatively = by a mean of 26.9 m in the GH group compared with a 19.5 m reduction in the= placebo group (P < 0.05). Dose-related side-effects were seen in the major= ity of patients receiving GH. CONCLUSIONS: In elderly patients undergoing t= otal hip replacement, preoperative GH treatment results in improvements in = lean body mass and skeletal muscle mass that are sufficient to offset posto= perative losses. A one group pre- and post-test design was used in this study. Nurs= es, dietitians, and physical education teachers worked collaboratively to p= rovide a series of comprehensive, integrated education programs. Course con= tent included healthy life style and health promotion, disease prevention, = nutrition, exercise, and medication education. A total of 140 elderly parti= cipated in this study. Ninety- seven subjects attended all of the education= programs. A structured questionnaire was used for data collection. Informa= tion about demographics, health status, health promotion knowledge and beha= viors was included. The health promotion behavior data were collected twice= . The initial data set was collected prior to the first course and the seco= nd after the fifth course. Health promotion knowledge was assessed pre- and= post-test in the second, third, and fourth courses. The research findings = revealed that the education programs were effective in improving elderly he= alth promotion knowledge and behaviors. The scores for health promotion kno= wledge and positive health behaviors were high among subjects who were aged= 65-69 years, were married, lived with family members and had higher educat= ion levels. DESIGN: Prospective, randomized controlled trial. SETTING: Universit= y Medical Center and Veterans Affairs Medical Center, Taipei, Taiwan. SUBJE= CTS: Thirty-two of 64 patients with Fontaine stage II peripheral arterial o= cclusive disease (PAOD) were randomized to exercise training and 32 to usua= l care control. Five patients from the exercise group and six patients from= the control group dropped out, leaving 27 and 26 patients, respectively, c= ompleting the study in each group. INTERVENTIONS: Twelve weeks of treadmill= exercise training. MAIN OUTCOME MEASURES: Treadmill walking time to onset = of claudication pain and to maximal claudication pain, 6-min walk distance,= self-reported ambulatory ability and perceived health-related quality of l= ife (QOL). RESULTS: Compliance of exercise programme was 83% of the possibl= e sessions. Exercise training increased treadmill walking time to onset of = claudication pain by 88% (P < 0.001), time to maximal pain by 70% (P < 0.00= 1), and 6-min walk distance by 21% (P < 0.001). SUBJECTS: Perception of hea= lth-related QOL improved from 12% to 178% in the exercise group. These impr= ovements were significantly better than the changes in the control group (P= < 0.05). CONCLUSIONS: Significant improvements in claudication following 1= 2-week exercise training in elderly PAOD patients were observed.
